Beverley Dental Family Benefits

Early detection and prevention of dental diseases.
One (Silver) or Two (Gold and Children) examinations annually with our experienced dentists.
Two hygienist appointments.
5% (Silver) or 10% (Gold) Discount on routine restorative care.
Free Small Film X-rays.
20% Discount on tooth whitening*
5% discount for treatment that requires a dental technician (eg. Crown and bridgework)

Following the first appointment, the provision of routine examination and hygiene visits is subject to the receipt of the required consecutive monthly payments.
The membership commences on the date the first payment is due and continues until the patient cancels -giving us 30 days notice. The dentist can cancel the agreement giving the patient 30 days notice-or with immediate effect if the patient, giving their reasonable opinion, commits any act of omission (e.g. persistently failing appointments, not complying with practice recommendations).
Should the patient cancel or fail to pay membership within 12 months then any discounts allowed on procedures become payable in full. Any benefits from the plan must be used within a year and the benefits (including the examinations and hygienist visits) cannot be rolled over or reimbursed.
The following treatments are NOT included in the plan – Tooth Implant treatment/Orthodontics/Sedation/ Consumables/Treatment outside surgery hours.
If there is an existing special offer for any treatment a further membership discount will not be applied.
A maximum membership discount per calendar of £400 applies.
Membership fees are reviewed yearly.
The patient agrees to give the dentist at least 24 hours notice of cancellation. If the patient fails to give such notice or fails to attend, the dentist will not be able to reinstate the appointment.
